The initial inquiry report of PIMS incident has been given to Prime Minister Shehbaz Sharif. The investigation committee has determined the causes of the incident and the people responsible for it and recommended action against them.

Sources claimed the report was compiled by an investigative committee headed by former interior secretary Shahid Khan and handed over late Friday to the office of Prime Minister’s Adviser Tauqeer Shah. The committee also briefed officials on the report’s principal findings and recommendations.

All the members of the inquiry committee have signed the report but the authority to make it public has been left to the prime minister.

Sources said the inquiry also highlighted problems experienced by rescue crews during relief operations and reviewed the circumstances that contributed to the incident.

The committee has recommended disciplinary action against the relevant PIMS staff members who were absent at the time of the incident.

The paper also makes recommendations to prevent re-occurrence. Interviews with PIMS administration officials and doctors are also included, along with testimonies from fire brigade and rescue workers involved in the emergency response.

The inquiry committee also expressed worry about treatment procedures and poor facilities at PIMS saying the arrangements available were not up to the international standards.

Sources stated the committee has recommended that the prime minister take urgent action against anyone found guilty.

The prime minister is said to have decided to take action on the basis of the results of the probe and procedures against the concerned officials are likely to be initiated soon.

Sources in the inquiry committee also verified that the report had been properly sent to the Prime Minister’s Office.

A high-level inspection team will visit Islamabad’s Pakistan Institute of Medical Sciences (PIMS) on Saturday after a recent tragedy at the hospital, an administrative order said.

The directive added that the PIMS administration has cancelled the leaves of the leaders of all clinical and non-clinical departments and directed them to assure their attendance at the hospital on August 29.

The instructions were provided by the Executive Director, PIMS, and directed the department heads to ensure the timely attendance of the relevant workers during the inspection.

Officials also have been asked to keep all essential records and information complete and available at all times for the visiting team.

The order also said the inspection team is expected to carry out full review of clinical and non-clinical departments and get briefings from concerned officials on various issues.

PIMS authorities directed the officers and heads of the departments to extend full cooperation for inspection and be ready to produce the required records and information there and then.

The government gave no specifics on the composition of the inspection team.

The Provincial Disaster Management Authority (PDMA) Punjab has warned the eighth spell of monsoon rains and said majority of the districts will receive rainfall from August 29 to September 5.

Rain is anticipated in Rawalpindi, Murree, Galiyat, Jhelum, Chakwal, Talagang, Attock, Sialkot, Sheikhupura, Narowal and Gujrat, said a spokesman of PDMA.

Heavy rains are also expected in Lahore, Mandi Bahauddin, Gujranwala, Wazirabad, Hafizabad, Kasur, Okara, Nankana Sahib, Sargodha, Khushab, Mianwali, Noorpur Thal, Faisalabad, Jhang, Toba Tek Singh, Layyah, Bhakkar, Sahiwal and Pakpattan.

The PDMA cautioned that urban flooding could occur in Lahore, Rawalpindi, Gujranwala, Gujrat and Sialkot, and landslides were probable in Murree, Galiyat and other mountainous places. In streams and nullahs flash floods may also occur.

PDMA Director General Saif Anwar Jappa has asked the commissioners and deputy commissioners of the entire province to be on alert. He said that the district administrations have also been put on notice on the directions of Punjab Chief Minister Maryam Nawaz.

He ordered officials in key cities to take preventative measures in advance to deal with the potential of urban flooding. The departments of Health, Irrigation, Communications and Works, Local Government and Livestock have all been put on alert.

The PDMA head asked individuals to take precautionary measures during inclement weather and not to stay in dilapidated or mud-built houses and shift to safer places during high winds and storms.

Citizens were also told to refrain from unnecessary travel, take shelter during lightning and keep children away from low-lying areas, standing water, electricity cables and poles.

The PDMA also asked the residents to stay away from open spaces during thunderstorms and bad weather. In case of any emergency, the public can call helpline 1129 of the PDMA.
